Applicant must be 17 years of age with an official copy of the birth certificate, or driver’s license, social security card & two small photos.  An official copy of a High School Diploma, Transcript or GED or proof of a 10^th grade education (applicant with 10 credits must be prepared to take an entry exam to be considered). Other fees may apply.

A $25.00 money order payable to State Board of Cosmetology is due the time of enrollment to obtain a student license.
